To decide which you need to ask yourself two questions each time:\n<ul>\n<li>Is this case note relevant to the current situation?<\/li>\n<li>Will this case note affect how the reader continues care for the patient?<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p style=\"padding-left: 40px;\">If the answer to both this question is \u2018no\u2019, you can omit this information. But, if the answer to any one of these questions is yes, you should include this information somewhere in the letter.<\/p>\n<ul>\n<li>Make mental notes during your reading time as you read for your understanding.<\/li>\n<li>If you still have time left in your reading time.
