In India, two accreditations dominate the conversation – NABH and JCI.

Hospital administrators often ask me which one is better, but that’s the wrong question. What they should be asking is which one aligns with the hospital’s overall strategy. NABH is great for getting government backing and unlocking some benefits for you. JCI is more about opening up to international patients and getting some international recognition. They’re not rival ideas – they’re just different tools for different markets.

This guide aims to break down both accreditations, look at their standards and requirements and try to help administrators figure out which path ( or a combination of paths ) is best for their hospital.

Explore free Hospital Administration courses now!!

What is JCI Accreditation?

Joint Commission International, or JCI for short, is the international arm of the US-based accrediting body The Joint Commission. They’ve been doing this internationally since 1998, and have accredited healthcare organizations in over 80 countries around the world.

JCI is generally regarded as the gold standard for hospital quality and patient safety. The Gold Seal of Approval is something that international patients, global insurers, and medical tourism facilitators all look out for. For a hospital that’s targeting international patients – particularly from the US, Europe, the Middle East, and Southeast Asia – getting JCI accreditation is going to be the most credible signal out there.

Standards: As of January 1, 2025, JCI’s 8th Edition Accreditation Standards for Hospitals are in force. They cover a lot of ground, including patient-centered care ( rights, assessment, education, nutrition, anaesthesia ), healthcare organization management ( staffing, medication management, infection prevention, risk management, information security, facilities safety, emergency preparedness ) and a new chapter on global health impact that looks at things like environmental sustainability and climate resilience.

The Process: JCI accreditation begins with a self-assessment, then you’ve got to apply and provide documentation. An on-site survey with JCI surveyors can take anywhere from a few days to a week, and they’ll be talking to staff and patients, reviewing medical records, observing how things work and taking a close look at the equipment and infrastructure. You’ll need to reapply for accreditation every three years.

How Hard is it?: JCI is pretty resource-intensive. You’re looking at one to two years to get ready for it. They’ve also changed the rules so that now you’ve either got to be 100% compliant or not accredited at all – you need to meet at least 90% of the standards to be considered.

What is NABH Accreditation?

The National Accreditation Board for Hospitals & Healthcare Providers – which is a part of the Quality Council of India – is the country’s national healthcare accreditation body. It was set up back in the mid 2000s.

NABH’s standards are tailored to the kind of healthcare that’s available in India, and the kind of regulations and demographics you’re dealing with. And the good news is that they’ve got international recognition – ISQua ( the International Society for Quality in Health Care ) has accredited them.

Standards: The NABH 5th Edition has got 100 standards and 651 objective elements, split into ten chapters, and it uses a pretty detailed scoring system ( 1-5 ). The standards cover things like patient-centered care ( access, assessment, continuity, rights, infection control ) and organization-centered standards ( quality improvement, management responsibilities, facilities management, human resources, information management ).

Process: The JCI process gets underway with a self-assessment, and that’s followed by an application and getting all the paperwork in order. After that, one of the JCI surveyors will come to visit your facility and the whole thing takes anywhere from a week to a few days. They’ll be talking to staff, patients, and poring over medical records – quite a lot to get through. The visit’s over, and JCI hands out accreditation for a solid three years, meanwhile, you’ve got to keep up with annual monitoring and rounds of re-accreditation.

Difficulty: The trouble with JCI is that it can be really resource-intensive. Before you even start the process, you’re looking at a prep time of one to two years. On top of that, the 8th Edition of the standards ditched that ‘partially compliant’ label – you’re now either fully compliant or not compliant at all. And to get that tick of approval, you’ve got to hit at least 90% of the required standards.

What is NABH Accreditation?

The National Accreditation Board for Hospitals & Healthcare Providers – that’s NABH for short – is a bit of a sub-board of the Quality Council of India. It was started back in the mid 2000s to act as India’s own national body for health care accreditation.

NABH’s standards have been tweaked to suit Indian regulations, infrastructure and the kinds of patients they see. They’ve even got the seal of approval from the International Society for Quality in Health Care – so you know the standards are pretty much on par with the rest of the world

Standards: The NABH 5th Edition is broken up into ten chapters, a total of 100 standards and 651 separate bits of criteria. The way they score it is on a simple 1-5 scale. Patient-centered care and organization-centered standards are all covered here – we’re talking stuff like getting patients what they need, how to deal with infections, and how to make sure staff are all on the same page

Process: NABH accreditation involves the option of a pre-check before the final assessment. The team comes in, looks over your documented procedures, and checks to make sure you’re actually doing what you say you are. Accreditation lasts for four years, and they’ll come back for a surveillance visit at around 20-24 months in. NABH might also drop by unannounced – just to make sure everything’s still ticking over

Cost and accessibility: Getting accredited by NABH is going to set you back around USD 10,000-20,000 – that’s a fraction of what JCI will cost. And it’s no surprise that so many hospitals are getting on board – around 1,200 have NABH accreditation, whereas JCI has only about 50 in India.

Explore free Hospital Administration courses now!!

KEY DIFFERENCES BETWEEN JCI AND NABH ACCREDITATION

Dimension

JCI

NABH

Origin

Joint Commission (USA), global arm

Quality Council of India

Recognition

Global — 80+ countries

India-focused; ISQua-benchmarked

Cost

Premium; significantly higher

USD 10,000–20,000

Government Benefits

Limited for Indian schemes

PMJAY incentives, CGHS/ECHS empanelment

Best For

Medical tourism, international patients

Domestic patients, tier-2/3 hospitals, government schemes

Validity

3 years

4 years (full)

Preparation Time

1–2 years

Varies; more accessible for smaller facilities

JCI VS NABH: STANDARDS AND ACCREDITATION REQUIREMENTS

Both frameworks prioritize patient safety, but their structure and emphasis differ.

JCI’s 8th Edition is organized around patient-centered care and healthcare organization management, with International Patient Safety Goals (IPSGs) covering correct patient identification, effective communication, medication safety, safe surgery, and infection prevention. JCI accreditation alone does not fill beds.

NABH’s strength is its domestic relevance.
