- Candidates must apply online only through DHS Assam official website dhs.assam.gov.in
- Before applying, the candidates must keep a scanned image of their documents.
- Candidate should have a valid e-mail ID and Mobile Number is mandatory for registration and email ID and the given mobile number should be kept active. Directorate of Health Service Assam will send intimation regarding certificate verification and other important updates

Candidates are requested to fill in the DHS Assam online application form with the utmost care, as most of them likely no correspondence regarding change of details will be entertained.

Documents Required
The following documents in original will have to be produced at the time of document verification and photocopies of all certificates are to be submitted:
- 2 copies of recent passport size color photograph duly self-attested.
- Birth Certificate for age proof.
- Class-VW passed certificate.
- Caste certificate (wherever applicable)
- Permanent residentship certificate / Domicile certificate (if any)
- Person with Disability Certificate / Differently Abled Certificate (if any)
- Employment Exchange Registration Certificate. (h) Economically Weaker Section (EWS) Certificate (wherever applicable).
- Valid Driving License Certificate (wherever applicable).
Application Fees
Candidates appearing for Group 3 and 4 posts, there is no fee to be paid.
Educational Qualification
- A candidate must be Class-VIII passed from Government recognized school.
- For the post of Electrical Helper, Laboratory Attendant, and O.T. Attendant. a candidate must possess a training / experience certificate, in the respective field.
